Background: the current study investigated platelet rich fibrin and subepithelial connective tissue graft in preserving crestal bone and soft tissue around delayed implants. Methods: Twelve participants with thin gingival biotype, were randomly assigned in two equal parallel groups. Both received delayed implant, augmented by subepithelial connective tissue graft versus platelet rich fibrin. Crestal bone level was measured at 3, 6 and 9 months. Gingival thickness and keratinized tissue width were measured at baseline, 3 and 6 months, pink esthetic score at 6 months, pain, swelling and patient satisfaction were reported after 7 days and at the end of the follow up period respectively. Results: Both groups showed statistically significant improvement in all clinical outcomes at 3 and 6 months with no statistically significant difference at different observational periods. Subepithelial connective tissue graft showed statistically significant increase in gingival thickness and pink esthetic score compared to platelet rich fibrin after 6 months with no statistically significant difference regarding the crestal bone loss, keratinized tissue width, postoperative pain, swelling and overall patient satisfaction between both groups. Conclusion: Inspite that PRF showed less values of crestal bone loss yet both treatment modalities could not prevent the postsurgical crestal bone loss to occur, with no statistically significant difference between them. Subepithelial connective tissue graft is effective at increasing the gingival thickness around delayed implants. Clinical relevance: Both treatment modalities could be used safely around dental implant. Subepithelial connective tissue graft is more effective in thin phenotype for augmenting gingival thickness.

Objectives: To assess accuracy and reliability of linear measurements for analysis of CBCT superimposition and determine if linear measurements are enough without requiring advanced analytical tools. Methods: Pre and post-operative CBCT scans of 20 orthodontic cases were used for CBCT superimposition. The gold standard was linear CBCT measurements on the axial section of each separate scan (pre and post) before superimposition. Voxel based and point based superimposition were done using Dolphin 3D (Dolphin Imaging and Management, Chatsworth, California, USA), In Vivo (Anatomage, San Jose, California, USA), and Ondemand 3D (Ondemand 3D; Cybermed Co., Seoul, Korea). Analysis of the superimposition was obtained on axial section of the superimposed models. Results were compared to the gold standard using error assessment according to Dahlberg error, Bland &Altman method and intra-class correlation coefficient. Results: Dolphin side by side method showed 25.4% relative Dahlberg error (RDE), 4.28 limit of agreement (LOA) and 0.768 intra-class correlation coefficient (ICC). In-Vivo point method showed 26.75% RDE, 4.48 LOA and 0.87 ICC. Whereas In-Vivo volume showed 24.66% RDE, 4.15 LOA and 0.822 ICC. On Demand showed 28.44%, 4.78 LOA and 0.7 ICC. Intra observer reliability of all methods regardless of the observers are poor. Conclusion: There is weak accuracy of the linear measurements on axial plane of CBCT superimposed models and it is not recommended alone for analysis.

This study was conducted to evaluate the effect of Vitamin D (Vit D) supplementation to topical corticosteroids in the treatment of oral lichen planus (OLP) among postmenopausal women. Thirty post-menopausal women, their ages ranging between 45-65 years, diagnosed with any form of Oral lichen planus and presenting with serum 25 Hydroxyvitamin D levels below 30ng/ ml were randomly allocated into one of two groups (15 patients each) and treated as follows: The control group I, received topical cortisone alone; The experimental group II, who were treated with topical cortisone supplemented with Vit D every week for one month. Serum level of TNF-α, clinical and pain scores were carried out at the time of the initial visit and at 2, 4, and 6 weeks. Results showed improvement in clinical score. Both treatment modalities resulted in a statistically significant decrease in pain score, and TNF- α (P < 0.001) compared to the baseline values. There was no statistically significant difference between the two groups (P > 0.05) at any evaluation period for the pain score, while TNF- α showed statistically significant differences at 4 weeks only in favor to group II (P < 0.05). Conclusion: Within the limitation of this study, it can be concluded that Vit D supplementation to conventional therapy can help in treating OLP among post-menopausal women. Attention must be given to educate the women in this critical age to pay attention to Vit D deficiency to avoid such serious diseases.

Abstract . Background: The health of the gingival tissue has an important effect on the general health. Defensins are proteins found in humans as well as other vertebrates. They have antibacterial, antiviral and antifungal functions. Objectives: The purpose of the present study was to assess expression of beta defensin on the gingival epithelium in rats. Methods and materials: A 30 adult Sprague-Dawley male rats were randomly divided into 3 equal groups (n=10), 5 rats for experimental study and other fives as control within each group. Ligatures were placed in the inferior frontal group which acted as a gingival irritant. Scarifications were done after 3 days, one week and two weeks alternative. The levels of defensins were tested in the gingival tissue of all rats. Results: the expression of β defensin 2 showed that there is a statistically significant difference in immunolocalization of Defensin-2 percentage between the 1st group (after 3 days) and control one (P ≤ 0.05), while there is a highly statistical difference when comparing both the 2nd (after 1 week) and third group (after 2 weeks) versus control group (P ≤ 0.01). Conclusions: From the present work, it may assume that, the rat model may be a good system for experimental analysis of the innate immune response to the bacteria in the oral cavity, as well as the potential role of β -defensins in the host response to colonization

ABSTRACT Statement of problem: Mandibular overdentures assisted by mini-implants induce sever bone changes in posterior mandible. Purpose: This study aimed to evaluate the posterior mandibular ridges height changes using the digital panoramic radiographic technique under mandibular overdentures assisted by four mini-implants, in case of interforaminal distribution and in case of wide distribution after wearing overdentures for 6 years. Methods and material: The subjects enrolled for this study (n = 20). They were treated with maxillary complete dentures, randomly assigned into two equal groups: group І received mandibular overdentures assisted by four mini-implants distributed equally in the interforaminal region, exposed to immediate loading protocol. Group II received mandibular overdentures assisted by four mini-implants (two mini-implants inserted in canine regions and two mini-implants inserted in first molar regions) exposed to immediate loading protocol. Digital panoramic radiographic evaluation of posterior mandibular region was done for every patient of both groups, recorded at the time of mini-implants insertion and after 6 years. Results: There was statistically significant difference in bone heights changes between interforaminal and wide distribution groups after six years. There is bone apposition in the wide distribution at 15mm and 20mm distal to mental foramen. Conclusion: Posterior mini implants placement resulted in improved denture support and minimal overdenture displacement. The functional load on posterior mini implants has a positive effect on preservation of the residual alveolar ridge. Wide mini implants distribution is an acceptable alternative prosthodontic design to four-interforaminal mini implants distribution when anatomical and economical factors permit.

Background and Objectives: This study was to evaluate referring dentists’ satisfaction with Cone Beam Computed Tomography (CBCT) reports prepared at the Department of Oral and Maxillofacial Radiology (OMFR) at the University of Connecticut Health Center (UCHC). Material and methods: This study was an anonymous survey of 150 UCHC dental faculty and residents and was approved by our IRB. The survey consisted of 23 questions asking respondents whether they had ordered CBCT studies, and if so, their impressions of the quality and usefulness of the reports and images sent back from OMFR, and their level of trust in the radiologist expertise. Questionnaires were distributed twice through participants’ school mailboxes with a one-week interval. Results: The majority of CBCT scans referrals (62%) were for implant treatment planning. Overall, 66.7 % of respondents were satisfied with CBCT reports. 73% indicated that the reports do not need modification, but a majority indicated that standardization of reports would be helpful. Conclusion: The use of CBCT scans are useful for the dentist. Most of the participants who had used that equipment proved that it should be allowed in that facility as it will improve their service delivery to the patients.

Abstract: The sella turcica is one of the most important landmarks used by orthodontics in analysis of growth and treatment planning. Therefore, the purpose of this retrospective study was to detect whether there is a correlation between the sella turcica's linear dimensions and the vertical growth patterns of the jaws using CBCT in an Egyptian subpopulation. 144 CBCT scans of adult patients (68 males and 76 females) were taken from the archive of the Oral Radiology Department , Faculty of Dentistry, Suez Canal University. CBCT images were divided according to the vertical growth pattern of both maxilla and mandible into three groups; hypodivergent, normodivergent and hyperdivergent. The sella turcica diameter, length and depth were measured individually for each case. There were no statistically significant differences found between sella turcica's length and diameter measurements and vertical growth pattern of the jaws. While significant difference was found regarding the depth only between the three groups where the p-value was found to be 0.001. Conclusion: CBCT could be used as a highly accurate radiographic modality in assessing the linear dimensions of sella turcica .The sellar depth was the only linear measurement found to be affected by the vertical growth pattern of the jaws.

Abstract: Background and Objective: periostin is a protein highly expressed within extracellular matrix of bone and periodontal ligament responsible for tissue integrity and play significant role in wound repair and bone remodeling. The present study designed to estimate the effectiveness of non surgical periodontal therapy (NSPT) on level of periostin in the gingival crevicular fluid (GCF) of diabetic patient with periodontitis. Materials and Methods: 36 subjects were admitted in this study. The subjects allocated into 3 groups, group I involve 12 periodontally healthy subjects, group II involve 12 participants with periodontitis. Group III: involve 12 participants with periodontitis diagnosed with diabetes mellites type II. Group II and III were received conventional periodontal treatment scaling and root planning (S&RP). Clinical parameters: pocket depth (PD), clinical attachment loss (CAL), gingival index (GI), plaque index (PI) were measured and the GCF samples were gathered for assessment of periostin at baseline and at 3 months postoperatively. Results: periostin level in the GCF were significantly increased in healthy subjects (P≤0.001) compared to those with diabetic patients with periodontitis and patients with periodontitis only. NSPT in group II and III resulted in significant increase (P≤0.001) in periostin level at 3 months postoperatively. Conclusion: NSPT can significantly increase the level of periostin in the GCF of diabetic patients with periodontitis and those with periodontitis only. Moreover, periostin suggested to be inflammatory marker reliable for diagnosis and evaluation of the outcome of periodontal treatment.

Abstract Background and Objective: Squamous Cell Carcinoma (SCC) of head and neck is a very common malignant tumor with high risk to develop in young individuals. Chemotherapy is limited treatment due to the side effects of the drugs and due to development of resistance to the drug; recently, it has been found that GSE has anticancer effects. Apoptosis is the cytotoxic effect that can be induced by extracts of medicinal plants, Active caspase-3 which is responsible for the morphological changes and fragmentation of DNA that occur during the apoptotic process. Methodology: Cytotoxicity of GSE was assessed on hep-2 cells using the MTT assay, the results were interpreted and half maximal inhibitory concentration (IC50) was estimated. Apoptotic cells death was measured using the Cell Death Detection ELISA kit, following treatment of cells with GSE for 24 h. Results: the cell viability percent showed a gradual decrease in a time- and dose-dependent manner. Drug treated cells revealed in microscopic examination nuclear morphological alterations correlated with apoptosis in its various stages, Detection of caspase 3 enzyme assays showed that in drug treated cells the level of caspase 3 increased with increasing concentrations of GSE. Conclusion: GSE induced the apoptosis of squamous cell carcinoma cells demonstrating the anticancer efficacy of GSE.

Context: Oral Mucositis (OM) is a very painful inflammatory condition, complicating patient’s life. Hence it was appealing to find natural remedies, dietary modifications or both to manage this condition successfully. Aim: to evaluate the efficiency of Propolis (P) and / or Intermittent Fasting (IF) in ameliorating OM. Materials & Methods: 40 male Albino rats weighing 200-250 grams, divided into: Negative Control Group (-ve control Gp): 8 rats kept healthy, Positive Control Group (+ve): 8 rats subjected to OM induction in their buccal mucosa, (P) Gp: 8 rats subjected to OM Induction, given Propolis orally, (IF) Gp: 8 rats subjected to OM induction & IF, (P&IF) Gp: 8 rats subjected to OM induction, maintained on both P &IF simultaneously. After three weeks, buccal mucosae were processed and stained by H&E, Masson’s Trichrome, and Anti-PCNA, examined by light microscope. Results: +ve control Gp showed significant increase in vacuolated epithelial cells, apparent decrease in immature collagen fibers in CT, and significant reduction in PCNA-positive cells. In the P, IF, and P&IF Gps, there were marked improvement in epithelium and CT. The (P&IF) Gp manifested maximum decrease in vacuolated epithelial cells, increase of immature collagen fibers, and maximum increase in PCNA positive cells. Discussion & conclusion: OM caused tissue destruction, which was evident in the +ve control Gp. Propolis intake greatly improved the histological appearance (confirming anti-inflammatory role of Propolis) as well as IF (confirming immune-boosting action). However, using both P & IF resulted in maximum improvement due to combined actions of both.

Introduction: One of the serious public health problems nowadays is the hepatitis C virus (HCV) infection. Hepatitis C virus infection morbidity is associated with a diversity of extrahepatic disorders counting the oral cavity. Some of the oral manifestations are oral diseases like lichen planus and xerostomia and side effects of the virus therapy. The aim of this study was to evaluate and compare the oral health status, of patients with and without virus C in a sample of the Egyptian population. Methods: Data were collected from the Gastroenterology department at Benha University. The study was designed as a case-control study; 39 Egyptian hepatitis C patients (Cases) and 75 Egyptian non-hepatitis C patients (Control). Oral examinations were done according to the modified WHO oral health survey 2013 criteria. Results: The prevalence of xerostomia was 69.2 % and 69.3% in cases and controls respectively, with no statistically significant difference between the two groups. Skin hyperpigmentation was 46.2%, 16% in cases, and control groups respectively with a statistically significant difference between the two groups. The least manifestation was lichen planus, recurrent aphthous ulcer (RAU), and candidal infection. Conclusions: Many changes in the oral cavity were found higher at the control group as compared to the case group except for the skin hyperpigmentation that was higher among the hepatitis C patients. Our findings reinforce other regional studies with a bigger sample in hepatitis C patients.

Abstract Background and Aim: Tumor growth is limited by the balance between the need for oxygen and nutrients and diffusion of these substances from vessels around tumors. Therefore, angiogenesis, development of new blood vessels from previous blood vessels, is necessary for tumor growth and metastasis. This study was carried out with the aim of evaluating the angiogenesis in oral cancer by assessing microvessel density (MVD) using Endoglin (CD105). This work also correlated the expressions of marker with histopathological grades of oral squamous cell carcinoma. Material and Methods: Immunohistochemical staining with Streptavidin peroxidase was used to analyze the CD105 expression in 25 cases of oral squamous cell carcinoma. Results: All studied cases showed positive of CD105 immunoreactivity. The study revealed a highly significant increase of CD105-MVD in poorly differentiated OSSC than well differentiated OSCC. Conclusion: Increased expression of CD 105 in high grade tumors suggests that Endoglin is likely to contribute significantly to resistance in response to therapy.

Chemotherapy tends to stop the growth and eliminate cancer cells even at distant sites. Most of them lead to adverse side effects and induce changes in normal tissues. The aim of the present study was to evaluate of apoptotic changes induced by 5-fluorouracil on the lingual mucosa and salivary glands of male albino rats through clinical, histological, immunohistochemical and histomorphometric assessments. Twenty male albino rats were divided equally into two groups. Control group did not receive any type of treatment and study group received intraperitoneal injection of 5-FU (150 mg/kg) for 7 days. On the 14th day, the tongue and submandibular salivary gland specimens were dissected and processed. The study group showed histopathological changes as atrophy of the lingual papillae which confirmed statistically through histomorphometric analysis. The submandibular glands showed marked degeneration in the duct system and acini. The assessment of apoptosis was confirmed by immunohistochemical findings, there was highly statistically significant increase in the expression pattern of Bax and Caspase-3 with significant decrease in PCNA level in comparison to the control group. So, 5-FU caused deleterious histopathological and immunohistochemical alterations of the submandibular salivary glands and tongue mucosa.

Background: Diabetes Mellitus causes many systemic and oral complications including impaired wound healing. Recently, platelet-rich plasma (PRP) treatment to utilize a patient’s own platelets and growth factors to improve healing at the site of injury has been used extensively. Objective: Evaluation of the effect of PRP on tongue wound healing in normal and Streptozotocin-induced diabetic albino rats by H&E and Masson's trichrome histological stains and by TEM ultrastructural examination. Methodology: A total number of 108 adult male albino rats with average weight 200gm, were used in the experiment. The rats were classified into two main groups: non-diabetic and diabetic groups. Each group was further divided into three subgroups: non-treated wound, PRP-treatment before wound, and PRP-treatment after wound. Tongue specimens were dissected on postoperative days 3, 7, and 10. The specimens were examined histologically by H&E and MT, and ultra-structurally by TEM. Results: The most rapid wound healing was revealed in the non-diabetic and diabetic subgroups treated with PRP before the wound, which occurred very early at the 3rd day postoperative. While complete wound healing was revealed at the 7th day postoperative in both the non-diabetic and diabetic subgroups treated with PRP after the wound, which was like the non-diabetic control subgroup. Whilst, the diabetic non-treated subgroup was the last to show wound healing which took place at the 10th day postoperative. Conclusion: PRP could be applied before or after wounding occurs, to prevent impaired wound healing and to enhance wound healing in diabetic and non-diabetic oral mucosal wounds.

Background: The extracted teeth have proven to be a ready source of bone substitute material and thus, they are no longer viewed as clinical leftover. Many techniques are employed to process the teeth into particles with different sizes, shapes and calcium composition. Aim: The aim of the trial is to compare autogenous tooth (AT) biomaterial and the autogenous decalcified dentin (ADD) biomaterial for their efficacy in socket preservation procedures in relation to the radiographic dentistry measured on cone beam computed tomography (CBCT) images. Methodology: A total of 12 hopeless teeth were randomly assigned into both study groups. After executing atraumatic extraction, the tooth was cleaned and milled then it was prepared into either AT or ADD with decalcification in hydrochloric acid with pH=1 then inserted in the extraction socket. Baseline CBCT and the final images taken after six months were compared for radiographic density expressed in Hounsfield units (Hu). Results: After six months, all sites achieved soft tissue closure and were filled with bone without incidents. Radiographic bone density values were 830.67±115.5 and 742.83±148.84 Hounsfield units for the AT group and the ADD group respectively without a statistical difference. Conclusions: Both types of biomaterials were similarly effective in bone healing after extraction by providing radiographic density in treated sites.

This study aimed to evaluate the anti-inflammatory effect of TQ-PLGA/PVA in treatment of oral carcinogenesis. Nanoparticles were fabricated via emulsification-solvent technique. Their characterization including the morphological analysis and size distribution on NPs performed using TEM and the structure composition was carried out via the FTIR. The charge determined via the DLS and ZP analysis. This study was carried out on 24 hamsters and divided into: group A served as negative control, group B was painted with DMBA for 6 weeks then subdivided into three subgroups: B1 served as positive control, B2 was injected with TQ, B3 was injected with TQ-PLGA. Assessment of the blood cells counts, histologically, immunohistochemically through detection of TNF-α protein were performed. Comparisons between groups were performed using ANOVA. The results showed proper encapsulation for TQ-PLGA, significant regression in tumors histologically, marked improvement in blood cells and expression levels of TNF-α protein. Conclusion: systemic application of TQ-PLGA was considered a promising chemo-therapeutic agent in tumors regression.

Objective: The aim of this study is to assess the effect of different CBCT voxel sizes on the soft tissue thickness measurements in a variety of maxillary and mandibular regions. Methods: Two thicknesses (less than 2.0 mm and more than 2.0 mm) of pink baseplate wax as simulated soft tissue were overlaid on a maxilla and mandible of a dry human skull. The thicknesses were measured with a digital caliper as the physical measurements. The skull was scanned using CBCT with four different voxel sizes (0.20, 0.25, 0.30 and 0.4 mm). CBCT measurements were compared to the physical measurements. Intraclass Correlation Coefficient was used to assess both intra- and inter-observer reliability. Significance was considered to be 5% (P ≤ 0.05). Results: Results showed a significant difference between the CBCT measurements using the 0.3 and 0.4 mm voxel sizes scans compared to the physical measurements when wax thickness was less than 2.0 mm. On the other hand, no significant difference was found for the CBCT measurements using the four voxel sizes when thickness was greater than 2.0 mm. Conclusions: CBCT scans with voxel sizes of 0.3 mm and 0.4 mm provide accurate simulated soft tissue measurements when the thickness is more than 2.0 mm. However, when greater visualization of details are required, scans with lower voxel sizes is required. Therefore, in a clinical situation, it is of great importance to customize the resolution protocol according to the accuracy needed.

Abstract: The aim of the present study was directed to investigate the effect of rosmarinic acid (RA) as a chemopreventive modality on experimentally induced hamster buccal pouch (HBP) carcinogenesis. Material and methods: Forty Syrian male hamsters five weeks old, weighing 80-120g, were divided into four groups of ten animals in each as follows, GI: Topical application of liquid paraffin alone (three times a week for 14 weeks), GII: Topical application of 7, 12 dimethyl benz[a]anthracene (DMBA) alone (0.5% in liquid paraffin, three times a week for 14 weeks), GIII: Topical application of DMBA (0.5% in liquid paraffin, three times a week for 14 weeks) + Oral administration of RA (100mg/kg b.w in 1ml distilled water by oral gavage, three times a week for 14 weeks on alternate days of DMBA application), GIV : Oral administration of RA alone (100mg/kg b.w in 1ml distilled water by oral gavage, three times a week for 14 weeks). Results: Gross observation and histopathological findings revealed a-GI: normal stratified squamous epithelium b- GII: well and moderately differentiated SCC c-: GIII: revealed considerably reversed tumor incidence d: GIV: normal similar to GI. Immunohistochemical results revealed, oral administration of RA to DMBA treated hamsters restored the normal expression of bcl-2 and Bax indicating that RA may have chemopreventive mechanism and/or pro-apoptotic effects. Conclusion: RA may act through various mechanisms, such as exerting anti-inflammatory, antioxidant effects as well as inhibiting cell proliferation, migration, and selectively inducing cancer cells apoptosis.
